Community Based Alternatives

CBA illustrates Texas’ commitment to the aged and disabled who would like to live at home or in an assisted living environment rather than in a nursing home. Elderly and disabled clients can receive attendant services, nursing care and therapies through this program. Medical supplies, durable medical equipment and home modifications also can be provided through CBA. A physician’s order is needed for participation in this program. Grace Visiting Nurses is committed to providing outstanding service to our CBA clients. Case management is provided by DHS, as well as Grace Visiting Nurse’s RN Case Managers.
